Please bring the following documentation to register a new student or kindergarten student:

Proof of age (i.e. birth certificate, hospital record)
Documentation of residency (i.e. drivers license, rental agreement, power bill)
Proof of immunization - Kindergarteners must have the following immunizations:

4-5 doses DTaP
3-4 doses Polio
3 doses Hepatitis B
2 doses MMR
1 dose Varicella or have had disease (chickenpox)
2 doses Hepatitis A

Immunizations are available from your health care provider or the following places:
