Trip Request


Here is a quick step by step review of the process:  Please open the new trip forms for 2017-18.

  1. Staff member initiating the trip fills out the form.
  2. Staff member will email form to Transportation Department and Their site administrator. 

Administrators granting approval authority to one of their staff members must confirm who they are giving such authority with both the Transportation and Business Departments.

It works best if you create a folder and save master form to that folder.  After you have a master you will not need to return to the Web page.                                                         

After you have completely filled out this form you will need to:                                                         

  1. Print a copy for yourself, and save to a trip folder.
  2. Confirmation will be sent back to you via E-mail.
  3. If you have changes after you submit your request, please email it to Tamura Cook: cookt@eaglepnt.k12.or.us
